Please only request companies you are almost certain to use within 6 months. Requesting companies you do end up using will only result in a lapse and make it harder to process again in the future.
* These companies have appointment fees. Fees will be charged either through automatic bank draft, through ASB statements or via credit card form that will be emailed to you. (Typical amounts are: AL $160, FL $60, GA $48)
